Physical – must be completed on the UW-Parkside form and must have completed no later than 3 months prior to classes starting. Must be completed by a Licensed Medical Doctor (MD) or Doctor of Osteopathy (DO)
Insurance and Medical Information on Homelink – each athlete will be given a password and login in information. Homelink is a trustworthy site ... It comes up restricted because the athletic training staff has access to it to retrive information.

Health History Questionnaire – Must be completed prior to the physical and signed by the physician completing physical form. All yes answers must be explained. Form is found the homelink system.
Authorization Form – Must be signed and returned prior to participation. Must also include copy of front and back of Insurance Card
