Prospective student-athletes who have been selected to tryout for UW-Parkside Athletic teams must be cleared by the UW-Parkside Sports Medicine Staff prior to engaging in any athletic activities at UW-Parkside.

To be cleared a prospective student-athlete must:

1. Complete and sign the tryout form (including sections on eligibility, emergency contacts, consent to treat and assumption of risk)
(if the prospective student-athlete is under the age of 18, a parent or legal guardian must also sign the form)
2. Complete official visit tryout pre-approval form.
3. Present sports physical examination administered within the last six months. ATT: Wisconsin high school student-athletes, alternate year physicals do not satisfy NCAA requirements for tryouts and prospective student-athletes will need to obtain a new physical that is within six months of the tryout date.
4. Present proof of current medical insurance (photocopy of front and back of insurance card)
5. In accordance with NCAA rules, all student-athletes must be tested for sickle cell trait. Before any prospective or current student-athlete will be permitted to try out, practice, compete, or take part in out-of-season training, the student-athlete must undergo a medical examination that includes a sickle cell solubility test, unless either: 1) document results of a prior sick cell test are provided by the student-athlete, or 2) the student-athlete declines the test and signs a written liability release.
